Geriatric Care Management

The American Hospital of Paris offers various types of care pathways, guaranteeing quality, multidisciplinary medical care delivered by professionals from multiple specializations. The following are several examples of the care pathways within the hospital that we offer our patients over 80 y. old

Orthopedic and digestive surgery: in the case of a planned or emergency orthopedic surgical procedure (i.e., hip fracture) or digestive surgery in a patient older than 80 with multiple chronic disorders, we offer evaluation and treatment by three professionals: a surgeon, an anesthesiologist and a geriatrician, in order to optimize treatment and reduce post-operative complications, especially confusion.

Cardiology: we offer a consultation meeting with multiple specialists, including a cardiologist, pharmacist and geriatrician, to offer the most suitable treatment (anticoagulent, auricle closure, abstaining from treatment if appropriate) for heart rhythm disorders in patients at risk for falls or anticoagulant-related bleeding.

Oncogeriatrics: after the referring oncology doctor, the oncology surgeon or the radiation therapist screens an elderly patient that has cancer or a hematologic disease to detect potential vulnerabilities, the oncologist, surgeon, radiologist, radiation therapist, internist and geriatrician hold a consultation meeting to propose an appropriate treatment with respect to the patient’s condition, to support the patient and to minimize the side effects of the potential treatments for the greatest possible benefit.

Complex behavioral problems: whether the patient is hospitalized through the emergency room or admitted directly into a room, besides a quick and thorough check-up (MRI or brain scan, EEG, DaT scan, lab tests), an evaluation by three specialists‒a neurologist, a psychiatrist and a geriatrician‒is provided for patients older than 80 who present complex behavioral problems which may be attributable to somatic and/or psychological causes. We also provide care for the primary caregiver in these situations.

Medication Reconciliation: patients with multiple chronic conditions often take a lot of medication. The primary goal of medication reconciliation is safe management of the patient’s medications. Our team of multiple accredited specialists (pharmacist, geriatrician, nurse, pharmacy assistant) works at the patient’s bedside and with those around the patient in an effort to prevent and detect potential medication errors (iatrogenic accidents).

For ambulatory patients, a geriatric consultation is offered.
